If you have a family member with a developmental disability in New Jersey, you’ve likely heard the term “DDD waiver” — but understanding what it actually means, what it covers, and how to access it can feel like navigating a maze. You’re not alone. Thousands of New Jersey families face this exact challenge every year.
At GoldenGreys Community Services, we work with NJ DDD waivers every day. This guide will walk you through everything you need to know in plain language — no government jargon, no confusion.
What is the NJ Division of Developmental Disabilities (DDD)?
The New Jersey Division of Developmental Disabilities (DDD) is a state agency within the Department of Human Services. Its mission is to promote the independence, inclusion, and self-determination of individuals with developmental disabilities by supporting them to live, learn, and work in the community.
DDD currently supports over 25,000 individuals and families across New Jersey through a range of programs and funding streams — the most significant of which are called Medicaid Home and Community-Based Services (HCBS) waivers.
What is a Medicaid Waiver?
A Medicaid waiver is special federal permission granted to states that allows them to use Medicaid funds for services that go beyond standard Medicaid coverage. In simple terms, it pays for community-based supports — things like personal care assistance, respite care, community activities, and skills training — that would otherwise have to happen in an institution.
💡 Key point: NJ DDD waivers allow individuals with developmental disabilities to live in their own homes and communities, supported by services funded through Medicaid. Without waivers, these services would not be covered.
The Three Main NJ DDD Waivers
1. The Supports Program Waiver
This is the most common waiver and is designed for individuals who need a moderate level of support to live in the community — typically in their family home or their own apartment. Services covered include:
- Individual Supports (personal care, daily living skills)
- Community-Based Supports (community outings, social skills)
- Respite Services (temporary caregiver relief)
- Prevocational Services (work-readiness training)
- Supported Employment
- Day Programs
GoldenGreys Community Services is an approved Supports Program Waiver provider.
2. The Community Care Waiver (CCW)
The Community Care Waiver is designed for individuals who need a higher level of support, including those who live in group homes or need 24-hour oversight. CCW services include everything in the Supports Program plus residential services and 24/7 support staffing.
3. The Consolidated Waiver
The most intensive waiver, designed for individuals with the highest level of medical or behavioral support needs. It is typically accessed through a specific eligibility process with DDD.
How Do I Know If My Family Member Is Eligible?
To be eligible for NJ DDD services, an individual must:
- Be a New Jersey resident
- Be at least 3 years old (with no upper age limit)
- Have a developmental disability as defined by DDD, including: Intellectual Disability, Autism Spectrum Disorder, Prader-Willi Syndrome, Cerebral Palsy (with intellectual disability), Spina Bifida (with intellectual disability), or other neurological impairments originating before age 22
- Meet Medicaid financial eligibility criteria
📋 Important: Eligibility and diagnosis documentation will be required. Start gathering medical records, psychological evaluations, and school records early in the process.
How to Apply — Step by Step
- Contact NJ DDD: Call the NJ DDD Central Office at 1-800-832-9173 or visit dhs.nj.gov/ddd to start an eligibility determination.
- Complete the Application: You’ll be asked to submit documentation of the disability diagnosis, proof of NJ residency, and Medicaid eligibility information.
- Eligibility Assessment: A DDD representative will review your application and conduct an assessment. This can take several weeks to months.
- Wait List or Direct Enrollment: Depending on the level of need and waiver availability, your family member may receive services immediately or be placed on a waiting list.
- Individual Service Plan (ISP): Once approved, you’ll work with a Support Coordinator to create an Individual Service Plan that outlines the specific services, goals, and providers.
- Choose a Provider: You have the right to choose your own DDD-approved service provider. This is where GoldenGreys comes in.
